Whose fault is that? The guard(s) for not properly distributing the ball, or Fletcher (and Pippen) for not working hard enough to get it? From what I've seen its a combination of both, and it's not consistent from game to game.
Well said BKnight. Good offensive players work to get open cause they Want the ball! Coaches also have to drill it in the point guard's head to get the ball in certain guys' hands. I've seen a couple of games where I thought Pippin was just going through the motions and not hustling.

Good question. Some on both sides I think. However, when the offense has your big man wandering around at 18 feet for a good portion of the time that is coaching and lack of a coherent offensive strategy.

Its a coaching / game planning issue.
More coaching than game planning. It's an issue that should have been taken care of in practice before December 1. Hinson hasn't done a great job of evaluating combinations, has not done a good job of breaking things down in practice for these kids, and frankly hasn't demanded enough effort out of them on a consistent basis. The effort of this team is inconsistent and frankly the basketball IQ seems to be limited.
The stat sheet for this team is quite telling. Hinson needs to look at it.
